This is a documentation workbook, not a government portal, legal service, fraud determination, or recovery service. It does not submit reports or guarantee acceptance or recovery.

Keep sensitive information out

Do not enter Social Security numbers, full card or bank account numbers, passwords, PINs, authentication codes, or private keys. Use descriptive account labels and last four digits only.

Five-step evidence sequence
  1. Stop further loss first. Contact the bank, card issuer, payment service, or relevant provider before documentation work.
  2. Write five facts. Note what happened, when it began, how contact occurred, how money or access moved, and what remains at risk.
  3. Build the timeline. Record events in time order without guessing at missing details.
  4. Preserve original evidence. Keep receipts, messages, screenshots, account names, and file locations in their original form when possible.
  5. Record official contacts. Save provider contacts, report numbers, dates, and next steps before creating the printable report.
Helping a family member
  • Ask permission before organizing or sharing their information.
  • Let them describe events in their own words without blame.
  • Separate what they told you from what you personally observed.
  • Use family helper mode below to identify who entered or verified information.
